Preparation for the examination
General
- The material for testing is blood.
- The patient does not need to fast. It is recommended to drink a glass of water approximately 30 minutes before the examination.
Description
The test is used to
aid in the diagnosis and treatment of vitamin B12 (cobalamin) deficiency.
Vitamin B12 (cobalamin) in serum is bound to two
proteins: transcobalamin (TC) and haptocorrin (HC). The complex
of transcobalamin and vitamin B12 is called holotranscobalamin
(HoloTC). Holotranscobalamin (HoloTC) contains biologically active
cobalamin, as only holotranscobalamin catalyzes
the uptake of cobalamin by all cells via specific
receptors. In comparison, approximately 80% of
haptocorrin-bound cobalamin is considered metabolically inactive due
to the lack of cellular receptors, except those found
in the liver. Genetically determined haptocorrin deficiency is
rare and not considered a serious condition. On the other
hand, a genetically determined lack of transcobalamin or its
abnormal values manifest themselves as typical
hematological, neurological, and
metabolic pathological conditions caused by cobalamin deficiency, requiring
radical treatment even when
serum cobalamin levels are normal.
The shorter half-life of circulating holotranscobalamin compared
to holohaptocorrin (HoloHC) makes a decrease in
holotranscobalamin concentration one of the first indicators
of cobalamin deficiency. Measurement of total cobalamin concentration in serum is limited
by certain factors, particularly the fact that most of the measured
cobalamin is bound to haptocorrin.
