Blood Group Identification Card

• To receive a Blood Group Identification Card, you will need:
  an ID card and two blood group test results (at least one test must be performed at the Blood Group Serology Laboratory at LUXMED CM in Lublin).
  If one of the results was performed in a different laboratory, it can only be accepted if it includes the following legible data:
  1. Patient's name and surname.
  2. PESEL number.
  3. Name of the unit performing the test.
  4. Blood group determined in the test.
  5. Date of test and test number.
  6. Signature of the person performing the test.
  The blood group entry in the hospital stay information card cannot be considered a test result.
  We do not accept blood group test results performed on newborns using umbilical cord blood immediately after birth.
  If the patient does not have blood group results and wishes to have two tests performed at the LUXMED CM Serology Laboratory, a blood draw for two tests is possible.

• For children, an ID card (ID or passport) is required to receive a Blood Type Identification Card. If the ID card is a passport, the child's parents' names must be included on the blood type referral. Children must bring their legal guardian with them to the blood test. The guardian's ID card is also required. Cards are issued to children over two years of age.

The Card is collected at the facility where it was commissioned.
The Blood Type Identification Card is only accepted with an ID document.

The card replaces the blood type entry on an
ID card. Once issued, it is valid for life.

It has the format of a credit card. It perfectly fills the gap created by the replacement of old, paper IDs with new, plastic ones.

The card contains the same personal data as the patient's ID card. If personal data changes, the card must be reissued.
