Description
Antibodies against desmosomes of the stratum spinosum and basement membrane of the epidermis are assessed using indirect immunofluorescence.
These antibodies are found in autoimmune dermatoses characterized by the formation of blisters on the skin.
Pemphigus vulgaris is a severe disease of the skin and mucous membranes covered by squamous epithelium, characterized by profound acantholysis.
The first symptom is often blisters in the oral cavity, which later also appear on the skin. Because these blisters rupture easily, painful erosions develop.
If left untreated, lesions spread throughout the skin. The presence of IgG antibodies against desmosomes of the stratum spinosum confirms the diagnosis (90% incidence).
The antibody titer correlates with disease activity. These antibodies are also detected in rare cases of burns and drug eruptions, but without clinical manifestations.
Pomphigoid bullosus is usually characterized by itchy blisters on the skin and often inflamed skin without involvement of the mucous membranes.
Patients are usually over 70 years of age.
Pemphigus and pemphigoid rarely coexist. Complications of these conditions include electrolyte disturbances and secondary infections.
