Description
Endomysium (reticulin) is a delicate, fine-fibered connective tissue surrounding muscle fibers.
In gluten-intolerant patients, ingestion of gluten-containing flour products causes damage to the mucosa of the small intestine.
This leads to atrophy of the intestinal villi and functional impairment. The clinical picture is characterized by diarrhea and consequences of malabsorption (weight loss, growth retardation in children).
Anti-gliadin and anti-endomysium antibodies are tested to confirm the diagnosis of gluten-sensitive enteropathy (celiac disease in children, nontropical sprue in adults) and Duhring's dermatitis herpetiformis (a chronic skin condition with blistering).
It should also be performed in blood-related patients with celiac disease to detect a predisposition to gluten-sensitive enteropathy.
Evaluation of these antibodies is also useful for disease monitoring, gluten-free diet monitoring, and gluten tolerance testing.
Antibodies against gliadin and endomysium of the IgA class are practically absent in healthy individuals and those suffering from other intestinal diseases, and their prevalence among untreated individuals with gluten enteropathy is almost 100%.
