Description
Endomysium (reticulin) is a delicate, fine fibrous connective tissue that surrounds muscle fibers.
In gluten-intolerant patients, consumption of gluten-containing flour products causes damage to the small intestinal mucosa.
Atrophy of the intestinal villi and functional disorders occur. The clinical picture is characterized by diarrhea and sequelae of malabsorption (weight loss, growth retardation in children).
Determination of antibodies to gliadin and endomysium is performed to confirm the diagnosis of gluten enteropathy (in children- celiac disease, in adults- non-tropical Sprue) and Dermatitis herpetiformis Duhring (a chronic skin disorder characterized by blister formation).
It should also be performed among hemorrhagic patients with celiac disease to detect a predisposition to gluten enteropathy.
Assessment of these antibodies is otherwise suitable for monitoring the course of the disease, surveillance of a gluten-free diet and gluten load test.
Antibodies to gliadin and endomysium of the IgA class are practically absent in healthy people and those suffering from other intestinal diseases, and their prevalence among untreated people with gluten enteropathy is close to 100%.
