Troponin I (high-sensitivity cardiac troponin) is the preferred biomarker in the diagnosis of myocardial infarction due to its high tissue specificity for the myocardium as well as its high clinical sensitivity. Troponin I is a protein found in the myocardium and, in conjunction with troponin T and troponin C, regulates the calcium-dependent interaction of actin with myosin. Three isotypes of TnI have been identified: one associated with rapidly twitching skeletal muscle, one associated with slowly twitching skeletal muscle, and one associated with cardiac muscle. The cardiac form contains an additional 31 amino acid residues in the N-terminal fragment of the molecule. Clinical studies have shown that cTnI is detectable in circulating blood 4-6 hours after acute myocardial infarction (AMI) and remains elevated for the next several days.
This increase in cTnI levels covers the diagnostic window for both CK-MB and lactate dehydrogenase. Subsequent clinical studies have shown that cTnI has greater specificity for myocardial injury than CK-MB. High-sensitivity cardiac troponin test results, combined with clinical and diagnostic test results, can be used to aid in assessing the risk of cardiovascular disease, including cardiovascular death, myocardial infarction, coronary artery disease, heart failure, or ischemic stroke in asymptomatic individuals. They can also be used to assess response to statin therapy.
