Troponin I is a protein found in cardiac muscle and, in conjunction with troponin T and troponin C, regulates the calcium-dependent interaction between actin and myosin. Three isotypes of TnI have been identified: one associated with rapidly contracting skeletal muscle, one associated with slowly contracting skeletal muscle and one associated with cardiac muscle. The cardiac form contains an additional 31 amino acid residues in the N-terminal fragment of the molecule. Clinical studies have shown that cTnI is detectable in circulating blood 4-6 hours after acute myocardial infarction (AMI) and its concentration remains elevated for the next few days.
This increase in cTnI concentration covers the diagnostic window for both CK-MB and lactate dehydrogenase. Subsequent clinical studies have shown that cTnI has a higher specificity for myocardial injury than CK-MB.
