hs Troponin I(high-sensitivity cardiac troponin)is the preferred biomarker for the diagnosis of myocardial infarction due to its high myocardial tissue specificity, as well as its high clinical sensitivity. Troponin I is a protein found in the myocardium and, in conjunction with troponin T and troponin C, regulates the calcium-dependent interaction of actin with myosin. Three isotypes of TnI have been identified: one associated with rapidly contracting skeletal muscle, one associated with slowly contracting skeletal muscle and one associated with cardiac muscle. The cardiac form contains an additional 31 amino acid residues in the N-terminal fragment of the molecule. Clinical studies have shown that cTnI is detectable in circulating blood 4-6 hours after acute myocardial infarction (AMI) and its concentration remains elevated for the next few days.
This increase in cTnI concentration covers the diagnostic window for both CK-MB and lactate dehydrogenase. Subsequent clinical studies have shown that cTnI has a higher specificity for myocardial damage than CK-MB. High-sensitivity cardiac troponin results, combined with clinical and diagnostic findings, can be used to help assess the risk of cardiovascular disease including cardiovascular death, myocardial infarction, ischemic heart disease, heart failure or ischemic stroke in asymptomatic individuals. They can also be used to assess response to treatment with statins.
