Issuing copies of medical records

Important information – please read before submitting your application

  • Requests for copies of medical records are accepted via the e-Form or directly at the Registration Desk.
  • A copy of your medical records will usually be prepared within 1 business day of your request.
  • Documentation can be collected at any Luxmed Medical Center branch. Copies of medical records are not sent by traditional mail to the address provided by the patient due to the inability to verify the recipient's identity and to ensure confidentiality.
  • A copy of the medical records is sent by email to the email address provided by the Patient in the application, which contains a trusted signature in a PDF document.
  • A copy of the medical records will be issued to the Patient, the Patient's legal representative or an authorized person based on verification of the identity document.
  • The authorized person is a person holding a notarial power of attorney or a person indicated by the Patient in a declaration completed personally by the Patient in Direct Registration.
  • In accordance with applicable regulations on the protection of personal data and medical data, other types of authorization will not be honored due to the impossibility of verifying their authenticity.
  • A fee is charged for issuing a copy of medical records in accordance with Article 28 of the Act of 6 November 2008 on patients’ rights and the Patient Ombudsman.
