• Examination and issuance of a decision - for fitness tests: from 4 years of age.
• Examination and issuance of a certificate - Polish Deaf Sports Federation: from 4 years of age.
• Medical examination and issuance of a certificate: from 4 years of age.
• Issuance of a duplicate certificate: from 4 years of age.
• Issuance of a medical certificate – sports medicine clinic: from 4 years of age.

A visit for a patient under 18 years of age is only possible in the presence of a parent/legal guardian
