eWUŚ, or Electronic Verification of Beneficiary Eligibility , is a system that confirms a patient's eligibility to use public healthcare. It is updated continuously. When registering, simply provide your PESEL number and identify yourself with an ID document such as an ID card, passport, or student ID.
What should I do if the eWUŚ system does not confirm health insurance (yours or your family members')?
- Submit a written statement
By signing the declaration, you confirm your entitlement to healthcare services, which will enable you to receive care under your health insurance policy. You can use the appropriate online form:
- Provide proof of insurance –
You can provide documentation confirming your entitlement to benefits, such as a pensioner ID card, a certificate from your employer, or a current health insurance application.
- Explain your situation
Contact your employer to confirm that you are covered by health insurance and that health insurance contributions are being paid regularly. Also, make sure that your family (husband, wife, children) is registered for insurance. Then, forward this information to your National Health Fund (NFZ) branch.
How do I register a family member for insurance?
If you want to register a family member for health insurance, you must be insured yourself. Inform the payer (e.g., employer) about family members who will be covered by health insurance, such as a newborn child. Registering a child is only possible after assigning them a PESEL number. You have a limited time to do this – 7 days. If you change jobs, you must re-register family members for insurance with the new employer.
For people living in Poland who are not covered by universal health insurance and are not entitled to benefits, there is the possibility of using voluntary health insurance.
To do this:
- Registration – Visit the provincial branch of the National Health Fund or one of its delegations, depending on your place of residence.
- Completing the application – Complete the application for voluntary health insurance, which you can print and complete at home or at a branch. You can find the application form here: https://pacjent.gov.pl/sites/default/files/2021-10/Wz%C3%B3r%20wniosku%20o%20wolne%20ubezpieczenie%202020.docx
- Documents – If necessary, provide documents confirming your last insurance period.
- Signing the contract – Sign the voluntary health insurance contract.
- Fee – Pay an additional fee if your insurance coverage has been interrupted for more than 3 months.
- Documentation at ZUS – After concluding the contract, submit the appropriate forms to the Social Insurance Institution: – ZUS ZZA ( Registration for health insurance) and ZUS ZCNA (Registration of data on family members for health insurance purposes), if you are registering family members.
Both you and your registered family members are entitled to health insurance benefits from the date specified in the contract. This entitlement continues for 30 days after the contract is terminated or expires.
