To use primary health care at CM Luxmed, you must choose a doctor and a nurse .

Females, newborns and infants of both sexes – up to 2 months of age – are also covered by midwife care.

You can change your primary care physician/nurse/midwife no more than twice per calendar year . For each subsequent change, you pay an 80 PLN fee to the National Health Fund (NFZ). A change of primary care physician/nurse/midwife is not considered if the patient changes due to a move or if the selected primary care physician/nurse/midwife discontinues providing healthcare services.

Declaration for a newborn

Newborns up to 6 months of age, if they do not have a PESEL number, may be registered on the list of primary care physicians/nurses/midwives based on the insurance of the parent or legal guardian.

Once the child turns 6 months old, the National Health Fund (NFZ) IT system will verify the child's declaration as inactive. After the child turns 6 months old, the parent/legal guardian should complete the declaration by providing the child's PESEL number.

One declaration – full possibility of choosing a doctor

Patients register with their chosen doctor, nurse, or midwife for their children. However, this doesn't limit their choices – they can use the services of not only that particular doctor or nurse, but also any family doctor or pediatrician, as well as all nurses and midwives within the Primary Health Care program at Luxmed Medical Center.

HOW TO SIGN UP?

Declarations of choice of a primary care physician can be submitted:

• in paper form
• in any branch of CM Luxmed, the registration employee will print the declaration and give it to you for signature,
• at home, filling out as many copies as the number of family members to be transferred to primary healthcare. Then, with the completed and signed declarations, report to the selected Luxmed CM branch.

• in electronic form via the Online Patient Account (IKP):

 To do this, complete the appropriate application and sign it using a Trusted Profile or a qualified signature.

The process of logging in and completing the declaration on the website https://pacjent.gov.pl/

1. 1. Entering the website:
      • We go to the website https://pacjent.gov.pl/ .
   2. Login:
      • In the upper right corner, click the "Log in" button.
      • Log in to the system. If you don't have an account, learn how to set one up.
   3. Account access:
      • From the bookmarks bar, select "My account".
      • In the tab we look for the title "Your Primary Health Care (POZ)".
   4. Completing the declaration:
      • Click "Fill out declarations".
   5. Choosing a primary care physician and completing the e-declaration:
      • Once you have the opportunity to complete three e-declarations, you will begin by selecting a primary care physician.
      • In the "Doctor" field, click complete the e-declaration.
      • Remember to choose a doctor, a community nurse and, in the case of women, a midwife for comprehensive medical care.
   6. Updating personal data:
      • We check and complete our data: name and surname, PESEL number, address.
   7. Selecting a facility:
      • Select the branch. Enter the exact street address – Province: Lubelskie, City: Lublin, Street: Radziwiłłowska, Koncertowa, or Orkana.
      • Facility: LUXMED MEDICAL CENTER LIMITED LIABILITY COMPANY (PRIMARY HEALTH CLINIC)
      • Doctor: select from list.
   8. Adding a nurse and midwife:
      • Facility: LUXMED MEDICAL CENTRE LIMITED LIABILITY COMPANY (COMMUNITY AND FAMILY NURSE CLINIC)
      • Nurse: select from list.
      • Facility: LUXMED MEDICAL CENTRE LIMITED LIABILITY COMPANY (COMMUNITY NURSE AND MIDWIFE CLINIC)
      • Midwife: select from list.
   9. Signing and sending the declaration:
      • After filling in all the fields, click on the right side "Sign e-declarations".
      • The declaration will be signed using a Trusted Profile or a qualified signature.
      • Verify the document and click "send declaration".

Completing the declaration:

Now you can use Primary Health Care at the Luxmed Medical Center.

On your IKP you will receive information about the acceptance of the declaration or its cancellation if the document requires supplementation or correction.
