My Health Program - Check what you will gain

How to take advantage of free tests?

Want to take care of your health but keep putting off tests? Now you can get them for free!
Take advantage of the "Moje Zdrowie" , which allows for a comprehensive health assessment as part of preventive care funded by the National Health Fund.

Who is this program for?

The program is available to people registered at a primary health care clinic at the age of:

  • from 20 to 49 years of age – once every 5 years
  • over 50 years of age – once every 3 years

What research does it cover?

Basic research package (for each balance sheet item):

  • Blood count
  • Blood glucose
  • Creatinine with eGFR
  • Lipid profile
  • TSH
  • General urine test

For patients with risk factors, tests (based on the survey) are also planned as part of the extended package:

  • ALAT
  • ASPAT
  • GGTP
  • Total PSA in men
  • Anti-HCV
  • Lipoprotein(a)

How to use the program at CM Luxmed?

You can have the tests done free of charge if you have submitted a POZ declaration at CM Luxmed.

1. Complete the survey

✔️ on the IKP website (Internet Patient Account),

✔️ in the mojeIKP mobile application,

✔️ directly at your primary healthcare facility : ul. Koncertowa 4D, ul. Orkana 7, ul. Radziwiłłowska 5.

  • The completed survey will be accepted for implementation within no more than 30 days.
  • During this time, a medical professional will contact you to issue a referral for laboratory tests and schedule a follow-up appointment.

 

2. Do your research

Get your tests done before your follow-up appointment.
You can get your tests done at all LUXMED CM facilities and collection points.

 

3. Summary visit
Based on the test results and the interview with the patient, a health assessment will be made and an Individual Health Plan - you will learn what you can do for your health.
