It is valid for 30 days from the date of issue. During this time, the patient should register for the procedure.
Treatments are performed in Lublin, ul. Zwycięska 6A, and in Chełm, pl. Kupiecki 12.
Systemic rehabilitation at the Day Center
- Patients are referred to rehabilitation at the Day Centre (depending on the type of rehabilitation, the patient's age and their condition) by primary care physicians or specialists from hospital departments or specialist clinics.
- An e-referral to the Day Center is required.
- The care plan at the Center is determined by a specialist in medical rehabilitation at CM Luxmed.
- Rehabilitation at the Day Centre lasts from 15 to 30 treatment days, during which an average of 5 treatments are performed per day.
