Description of the study
Anal fissure treatment with botulinum toxin injection involves injecting a botulinum toxin preparation into the internal sphincter muscle on both sides of the fissure. This reduces the tension in the internal anal sphincter muscle and improves its local blood supply, resulting in healing of the anal fissure. This treatment can be used as a first-line method for healing an anal fissure, or as a second-line method when local pharmacological treatments such as suppositories and ointments are ineffective. The maximum effect of botulinum toxin is achieved 7-14 days after the procedure. The effect lasts for approximately 2-3 months.
Indications
- anal fissure
Contraindications
- hypersensitivity to botulinum toxin preparations,
- inflammatory and purulent changes in the anal canal,
- coexistence of perianal fistula,
- anal scarring.
Caution should be exercised in patients:
- with peripheral motor neuropathies (e.g. amyotrophic lateral sclerosis or motor neuropathy),
- with subclinical or clinical symptoms of neuromuscular conduction abnormalities (e.g. myasthenia gravis or Lambert-Eaton syndrome),
- taking aminoglycoside antibiotics,
- relatively patients taking oral antidiabetic drugs.
