How to get a health booklet
- Come to any Luxmed facility and collect 3 tubes for testing for SS carriers in stool – the cost of 3 samples is PLN 117.
- Get a chest X-ray (no need to book an appointment in advance) – cost PLN 50.
- On the day of the third swab collection, return the tubes to the facility – see opening hours below.
- After 3 business days, you will receive an SMS informing you that your result is ready and you can make an appointment with a doctor at any location by entering "health booklet" in the e-booking - cost PLN 60.

- Get a chest X-ray (no need to book an appointment in advance) – cost PLN 50.
- Make an appointment with a doctor at any location by entering the "health booklet" in the e-booking form – cost PLN 60.
* The X-ray laboratory at 5 Radziwiłłowska Street is open with a break between 1:30 p.m. and 2:30 p.m.

- Book an appointment with an occupational medicine doctor at the facility at Królewska Street 11 by entering "health booklet" in the e-Reservation.
- Please report to the facility 45 minutes before your booked appointment time to have a chest X-ray performed.
