An employee reporting for a preventive examination should complete a special questionnaire in which they provide information on, among other things, their current health status, professional career, past illnesses and treatments.
If the questionnaire is not completed before the visit, it will be completed at the doctor's office. This significantly impacts the duration of the visit and, consequently, unnecessarily prolongs the time needed for preventive testing.
The link to the survey is sent to the patient via SMS or email after the scheduled examination date.
