An employee reporting for a preventive examination should have a special questionnaire filled out, in which he provides information on, among other things, his current state of health, work history, past illnesses or treatments.
If the questionnaire is not completed before the visit, it is completed in the doctor's office. This greatly affects the time of the visit, and consequently makes the time needed for preventive examinations unnecessarily longer.
