We hereby inform you that due to the entry into force on 1 April 2015 of the Regulation of the Minister of Health of 26 March 2015 amending the regulation on conducting medical examinations of employees, the scope of preventive health care for employees and medical certificates issued for the purposes provided for in the Labor Code (Journal of Laws of 2015, item 457), the template for referrals for medical examinations (initial/periodic/control) has changed.
Taking the above into account, we kindly ask you to issue referrals for preventive examinations for your employees according to the template constituting Annex No. 3 to the above-mentioned Regulation of the Minister of Health.
