We would like to kindly inform you that due to the entry into force on April 1, 2015. Ordinance of the Minister of Health dated March 26, 2015, amending the Ordinance on the conduct of medical examinations of employees, the scope of preventive health care over employees and medical certificates issued for the purposes provided for in the Labor Code (Journal of Laws of 2015, item 457), the pattern of referral for medical examinations (preliminary/periodic/control) has changed.
Taking into account the above, please issue referrals for preventive examinations of your employees according to the model attached as Appendix No. 3 to the above-mentioned decree of the Minister of Health.
