The eWUŚ, or Electronic Verification of Beneficiary Eligibility, is a system that confirms a patient's eligibility for public health care. It is updated on a regular basis. During registration, all you need to do is enter your PESEL number and identify yourself with an identity document, such as an ID card, passport or school ID.
What if the eWUŚ system does not confirm health insurance (yours or your family members')?
- Submit a written statement
By signing the statement, you are confirming your right to health benefits, which will enable you to receive care under your health insurance. You can use the corresponding online form:
- Present proof of insurance -
You can provide documentation proving your eligibility for benefits, for example, a pensioner's card, a certificate from your employer, or a current application for health insurance.
- Explain your situation
Contact your employer to confirm that you are covered by health insurance and that health premiums are paid regularly. Also make sure that your family (husband, wife, children) is enrolled in insurance. Then submit this information to your NHF branch.
How do I report a family member for insurance?
If you want to report a member of your family for insurance, you must be insured yourself. Inform the premium payer (e.g., your employer) of the family members you want to be covered by health insurance, such as a newborn child. Enrollment of the child is possible only after the child has been assigned a PESEL number. You have a certain amount of time to do this - 7 days. If you change jobs, it is necessary to re-register family members for insurance with the new employer.
For those living in Poland who are not covered by universal health insurance and are not entitled to benefits, voluntary health insurance is available.
To do this:
- Notification - Visit the provincial branch of the National Health Fund or one of its delegations, depending on where you live.
- Filling out an application - Fill out an application for voluntary health insurance, which you can print out and fill out at home or on-site at the branch. - Documents - If necessary, provide documents proving the last period of insurance.
- Signing the contract - Sign the contract for voluntary health insurance.
- Fee - Pay an additional fee if the break in insurance lasted more than 3 months.
- Documentation with Social Security - After concluding the contract, file the appropriate forms with the Social Security Administration: - ZUS ZZA (Notification of health insurance) and ZUS ZCNA (Notification of data on family members for health insurance purposes), if you are reporting family members.
Both you and enrolled family members gain the right to health insurance benefits from the date specified in the contract. This right continues 30 days after the date of termination or expiration of the contract.
