Postnatal care provided by a primary care midwife begins when the baby is born .
Requests for care may be made by hospital staff by telephone before the patient is discharged home, or by the patient herself.
The first patronage visit should take place within 48 hours after the patient leaves the hospital.
Patronage visits concern the care of the patient and the newborn and may take place at home or in a clinic.
A patient who has participated in the Active Mom childbirth class receives the midwife's telephone number before giving birth and can make an appointment with the midwife herself.
The patient is entitled to 4-6 visits from a primary care midwife (including lactation visits) up to 2 months after delivery.
